Speech-Language Therapy for Voice Disorders

When Your Voice Doesn't Do What You Need It To

Your voice is how people recognize you, and when it changes, it affects more than speech. A hoarse, weak, strained, or unreliable voice can make work calls tiring, phone conversations frustrating, and social settings something to avoid. You may find yourself speaking less, or worrying that people notice the difference.

A voice disorder is present when your voice quality, pitch, or loudness doesn't meet your daily needs. It doesn't require that others notice a problem.

A Quick Note on Voice Disorders

Voice problems fall into a few broad groups, and they often overlap:

  • Structural: physical changes to the vocal folds or larynx, such as nodules, polyps, swelling, or age-related changes
  • Neurogenic: nerve or brain-related causes, such as vocal fold paralysis, vocal tremor, or spasmodic dysphonia
  • Functional: voice problems that develop from how the voice is being used, even when the structures themselves look normal, such as muscle tension dysphonia and chronic vocal fatigue
  • Psychogenic: rarely, a voice change linked to emotional or psychological stress, which we would coordinate with a mental health professional

Overlap is common. Ongoing voice misuse, for example, can lead to physical changes in the vocal folds, which then make the voice harder to use.

Any voice change that lasts more than a few weeks should be checked by an ear, nose, and throat doctor (ENT). Voice therapy works best alongside a medical diagnosis, and some voice changes need medical attention first.

What Voice Disorders Can Look Like

No two people experience a voice disorder the same way. Depending on the cause, changes might include:

  • Hoarseness, roughness, or a raspy sound
  • A weak, breathy, or fading voice
  • A strained or tight feeling when speaking
  • Vocal fatigue, where the voice gets worse the longer you talk
  • Pitch that is too high, too low, or breaks unexpectedly
  • Loss of voice, or a voice that cuts in and out
  • Throat discomfort, or needing to clear your throat often

How Therapy Helps

Voice therapy is built around the specific cause and pattern of each person's voice difficulty, since voice disorders aren't one single presentation. Treatment may include:

  • Vocal hygiene and habit changes: identifying the daily habits, such as hydration, throat clearing, and vocal strain, that are helping or hurting your voice
  • Voice exercises: structured practice to strengthen the voice and improve coordination between breath and voice
  • Breath support training: using breath efficiently so the voice doesn't have to work as hard
  • Tension release techniques: easing muscle tension in the neck, jaw, and throat that interferes with voice
  • Carryover to real life: practicing in your actual speaking situations, such as meetings, phone calls, teaching, and noisy rooms
  • Coordination with your medical team: working alongside your ENT or other physicians, particularly before or after procedures
